Repeat Prescriptions
We recommend registration with My Health Online which provides patients with access online to order repeat medication items. Please see our My Health Online Page for more details.
Click here to download a Prescription Request Form which you should then return to the practice, either in person or by fax on 01446 793115
Prescription Requests for repeat medication can be made by returning the prescription counterfoil to Reception. Alternatively you can submit the online Repeat Prescription Form below. Please make sure all sections are completed correctly.
Please allow 48 hours (excluding weekends and Bank Holidays) to action your prescription.
If you routinely use the Rhoose or St Athan surgeries, please speak to the receptionist there.
For safety reasons please note we cannot take requests over the telephone.
Please note lost prescriptions are not routinely replaced.
Are you going away?
The Three Month Rule:
A maximum of three months medication can be prescribed in one year. If you are travelling abroad for a period longer than three months you are advised to register with a local doctor in the area you are residing. To find out more about The Three Month Rule please click here.
Medication Reviews
Your medication is reviewed annually or opportunistically. If you are due a medication review, tests may be required prior to your review so you may be invited to attend the surgery for example a blood test, await the results then to book in to see the doctor or practice pharmacist.
Please order your repeat medication only as needed and if you no longer take any of your medicines please let us know.
